- SITE USER AGREEMENT. The Precision Urodynamics, LLC/Mobile Urodynamics Group
Web Site is comprised of various Web pages operated by Precision Urodynamics, LLC/Mobile Urodynamics Group
or its affiliates or contractors (collectively, the
"Site"). The Site is offered to you
conditioned on your acceptance without modification of
the terms, conditions, and notices contained herein.
Your use of the Site constitutes your agreement to all
such terms, conditions, and notices. Additionally,
various Precision Urodynamics, LLC/Mobile Urodynamics Group
Web Pages may themselves contain additional terms that
govern particular features or offers (for example,

conditions, and notices contained herein conflict with
the Additional Terms or other terms and guidelines

("Content") are protected by copyright. Users
are prohibited from modifying, copying, distributing,

licensing, creating derivative works or using any

material on this Site for your personal, non-commercial

REMEDY IS TO DISCONTINUE USING THE SITE.

represent that you own or otherwise control all of the
rights to your Submission as described in this section
including, without limitation, all the rights necessary
for you to provide, post, upload, input or submit the
Submissions.

- TERMINATION/ACCESS RESTRICTION. Precision Urodynamics, LLC/Mobile Urodynamics Group
reserves the right, in its sole discretion, to terminate
your access to the Site and the related services or any
portion thereof at any time, without notice.
- GENERAL. This Site is designed for use in the
United States and is to be construed and evaluated
according to United States law. If you use this Site
from locations outside the United States, you are
responsible for compliance with any and all applicable
local laws. Precision Urodynamics, LLC/Mobile Urodynamics Group
makes no representations that the materials contained
within this Site are appropriate for locations outside
the United States. Use of the Site is unauthorized in
any jurisdiction that does not give effect to all
provisions of these terms and conditions, including
without limitation, this paragraph. Users of the Site
agree to submit to the laws of the United States and the
State of Washington, without regard to principles of
conflicts of laws contained therein. Users also agree to
submit to jurisdiction and venue in Thurston County,
Washington. You agree that no joint venture, partnership,
employment, or agency relationship exists between you
and Precision Urodynamics, LLC/Mobile Urodynamics Group
as a result of this agreement or use of the Site. The
Site is subject to existing laws and legal process, and
nothing contained herein is in derogation of Precision Urodynamics, LLC/Mobile Urodynamics Group's
right to comply with governmental, court and law
enforcement requests or requirements relating to your
use of the Site, or information provided to or gathered
by Precision Urodynamics, LLC/Mobile Urodynamics Group
with respect to such use. If any part of these Terms and
Conditions is determined to be invalid or unenforceable
pursuant to applicable law including, but not limited
to, the warranty disclaimers and liability limitations
set forth above, then the invalid or unenforceable
provision will be deemed superseded by a valid,
enforceable provision that most closely matches the
intent of the original provision and the remainder
hereof shall continue in effect. Unless otherwise
specified herein, these Terms and Conditions constitute
the entire agreement between the User and Precision Urodynamics, LLC/Mobile Urodynamics Group
with respect to the Site and your use thereof, and it
supersedes all prior or contemporaneous communications
and proposals, whether electronic, oral or written,
between the User and Precision Urodynamics, LLC/Mobile Urodynamics Group
with respect thereto. A printed version of these Terms
and Conditions and of any notice given in electronic
form shall be admissible in judicial, arbitral,
administrative or other proceedings based upon or
relating to these Terms and Conditions to the same
extent and subject to the same conditions as other
business documents and records originally generated and
maintained in printed form.
